Resili Health Expands View into Patient Data Using Mirth Health Data Hub

  • Resili Health helps healthcare organizations aggregate patient data and then use it to help clinicians provide better care. For example, data helps providers identify patients that are at risk for readmission so that they can modify treatment plans and avoid the need for readmission.

    To further ensure the data meets providers’ specific needs, Resili Health turned to Mirth Connect to leverage the solution’s flexibility to bring in, transform, and normalize data from their partner sites.

    To refine incoming data and formulate it for specific applications, the Resili team needed a more engineered ability to aggregate data from EHRs so their clients could see the entire patient journey through a hospital’s workflow.

    “The ability to view this aggregated, normalized patient information provides our partners and the clinicians with the information needed to support timely informed decisions,” said Nicole Bigelow, principal architect for Resili Health.

    The aggregated data set provides visibility of patient information across disparate data sources such that a provider can see a patient’s information from the hospital and beyond to where the patient was discharged for rehabilitation after surgery. This gives the provider insight to the post-acute care along with the ability to coordinate care with other providers that are treating the patient in the post-acute setting.

    This capability would also expand visibility into critical data that would support research and planning to improve clinical workflows.

  • “Mirth Health Data Hub overcame the challenge by providing a place to pull that data, aggregate it, master the patient identities, and then pull out parts of that data needed for different applications,” said Bigelow. “A longitudinal view into patients’ data provides a holistic view of the patient for physicians to improve their workflows and provide better care.”

    Resili Health selected Mirth Health Data Hub in part due to its cloud-based platform as a service architecture that allowed for scalability and growth to meet the performance and sizing needs. Other beneficial features included the integrated enterprise master patient index (EMPI) and the ability to store and re-order transactions even if they come in out of order.

    “There’s a bunch of positive things happening in Mirth Health Data Hub, and we wanted to take advantage of these capabilities,” said Bigelow.

  • Mirth Health Data Hub enables Resili Health to provide data to other downstream applications. This keeps care teams on the same page concerning the patient’s care. The solution’s performance management dashboards highlight analytics related to patient treatments. This data also helps clinicians stay informed of trends that could impact care.

    “Our primary use case is to take an aggregated set of patient data, take it apart, and provide it to other systems,” said Bigelow. “As a result, providers receive vital information that ensures informed decisions and makes workflows easier.”
